What is it?

Forms lets you build your own online enquiry forms — for example a booking request form for your website — and collect the answers as Requests. You can review each request, and turn it straight into a Proposal with the customer's answers already filled in.

You'll find it under Forms in the main menu, with two tabs: Forms and Requests.

How to

Create a form

  1. Go to Forms.
  2. Click Create new.
  3. Choose Start from scratch, or Use a template — such as Home cleaning, Moving cleaning, or Moving services — to begin with a ready-made set of questions.

Build the form

  1. Under Basics, set the Form name and an optional Intro text shown at the top of the form.
  2. Under Questions, click Add question in either Order details or Customer details, and choose one of the ready-made standard questions (like Order date or Address) or build your own.
  3. In the question's settings, add a Label, mark it Required if needed, and (for choice questions) add the answer Options.
  4. Under Terms, write your booking terms and turn on Require accept if the customer must tick a box to agree before submitting.
  5. Use the preview on the right to check how the form looks, and switch between Desktop and Mobile.
  6. Click Settings in the preview to set an Accent colour, a Thank-you message, and your Logo.

Note: Drag a question by its handle to reorder it, or use the duplicate and delete icons on each question.

Add follow-up questions based on an answer

  1. Open a Yes/No, single choice, multiple choice, or "Form type" question.
  2. Turn on Conditional follow-ups.
  3. Under each possible answer, click Add follow-up question.

The follow-up questions only appear to the customer if they pick that specific answer.

Publish and share a form

  1. Open the form and click Publish.
  2. Click Share to copy the public link or the embed code (to add the form to your own website).

Note: Sharing options only appear once a form is Active (published).

You can also click Share public page on the Forms list to get one link that lists every form you've published, so customers can choose the right one themselves.

Manage a form

  • Edit — click a form to open it in the builder again.
  • Duplicate — makes a copy you can adjust and publish separately.
  • Pause — temporarily stops new submissions without deleting the form.
  • Archive — stops the form from accepting new requests. Confirm to continue.

Review requests

  1. Go to Forms → Requests.
  2. Click a request to open it.
  3. Check the customer's answers under Customer and Service request, and open any Attachments they added.

Note: Hover over an answer to copy it — handy when you need to paste a phone number or address somewhere else.

Turn a request into a proposal

  1. Open the request.
  2. Click Create proposal.

A new proposal opens with the date, time, address, and other matching answers already filled in.

Note: This isn't available on the Basic plan.

If you already created a proposal for this customer another way, click Link existing proposal instead and search for it.

What do the fields mean?

Question types

  • Short text, Long text, Email, Phone number — simple text answers.
  • Number, Date, Time, Date range — numeric or date/time answers.
  • Yes/No, Single choice, Multiple choice, Dropdown — the customer picks from options you set.
  • File upload — lets the customer attach a file.
  • Address — a full street address.
  • Form type — a set of cards, pills, or tiles the customer picks one of, often used to steer which follow-up questions appear.
  • Label — not a question; a heading used to group and organise other questions.

Required

Marks a question with a red asterisk, so the customer can see it must be answered.

Fill on proposal

Links a question's answer to a specific field on a proposal (for example, order date or address), so it's filled in automatically when you create a proposal from that request.

Status (Forms)

Status

Meaning

Draft

Not yet published.

Active

Published and accepting submissions.

Paused

Temporarily closed to new submissions.

Archived

Closed permanently to new submissions.

Status (Requests)

Status

Meaning

New request

Just submitted, not yet looked at.

Under review

You're looking into it.

Proposal sent

A proposal has been created and linked to this request.

Note: A request can also show as Waiting for customer, Proposal created, Approved, Declined, or Archived, depending on what happens to the linked proposal — but these aren't statuses you set by hand.

Good to know

  • Forms doesn't have its own plan requirement, but converting a request into a proposal isn't available on the Basic plan.
  • Shared form links and embed codes open your form on your company's public booking page — the same system used for Eboka (Web orders).
  • There's no delete or reject action on a request — a request that shouldn't go anywhere just stays as New request or Under review.
  • This Requests tab, under Forms, only shows submissions to your forms — it's separate from any other "Requests" area you may see elsewhere in BOS.
  • Each standard question (like Order date or Customer address) can only be added to a form once.
